Family Connects is now offering visits to families with a newborn in Washington County born at Providence St Vincent Medical Center or OHSU Health Hillsboro Medical Center. To see if you are eligible for a no-cost visit, contact us at 503-846-5300 or FamilyConnects@washingtoncountyor.gov.

We are here to help because babies don’t come with instruction manuals! 

Most parents have questions or needs after bringing their baby home and that is expected. Our registered nurses are here for you. The visit includes a health check for mom and baby; tips and answers to your questions; and assistance with connecting you and your family to the things you need.

If you choose to schedule a Family Connects visit, a registered nurse visits* around 3 weeks after baby is born to check on mom, baby, and the whole family.

A Family Connects home visit includes:

For mom

• A health check

• Scheduling doctor appointments

• Talk about changes and things to expect

For baby

• Baby weight and health check

• Support with infant feeding, bathing, crying, sleeping and swaddling

• Talk about what to expect as baby grows

For family

• Exploring childcare options

• Home safety, parenting, and resources

• Playgroups and parent support groups

• Community connections as your baby grows
