The Lane County Family & Child Health is excited to meet your family wherever you are and connect you with the community resources to start your family off on the right foot! While we have not launched Family Connects visits quite yet, we continue our decades-long programs serving families on OHP through a variety of home visiting programs, including Nurse-Family Partnership, Healthy Families, and Babies First.

While we expect to launch Family Connects Oregon services in late 2024, families on OHP can already be served in our existing, long-standing home visiting programs.

Once launched, Family Connects Oregon services will be offered at the local birthing facilities by bedside screeners. Providers will also be able to refer clients, and our existing self-referral online form will continue to be used. You can find it at lanecountyor.gov/fch

Lane County Family & Child Health has two advisory boards- one of service providers, and one of families. We are always happy to have current or recently graduated clients join our FABulous Family Advisory Board.
