Lincoln, Benton, and Linn Counties
We were all babies once, and we want to help your baby get off to a good start! Family Connects Oregon will bridge the gap between families and the community, helping the family to get resources and services that they will need for their new family.
FCO of Malheur County provides Public Health Nurse home visits to families in Malheur County Oregon who are welcoming a new baby. A visit is scheduled around 36 weeks gestation for a visit to commence around the 3-week age of the baby. A nurse will assess both you and your baby, answer questions that you may have, and provide connection to local resources.
Family Connects is now offering visits to families with a newborn in Washington County born at Providence St Vincent Medical Center or OHSU Health Hillsboro Medical Center.

Family Connects is available to all families with newborns who reside in Polk County.

United Community Action Network (UCAN) will be providing Family Connects nurse home visiting services to all families with newborns who reside in inland Douglas County.

Family Connects Central Oregon provides Public Health Nurse home visits to families in Central Oregon who are welcoming a new baby. A visit is scheduled between birth and your first doctor’s visit and assesses mom and baby health, answer any questions you may have and provide connection to resources.
Family Connects is available to all families with newborns who reside in Yamhill County.

Bay Area Hospital will provide Family Connects nurse home visiting services in partnership with the South Coast Early Learning Hub. Services will start in Coos County, with roll out to Curry and Coastal Douglas over time.
